Summary

Combined ventricular assist device (VAD) implantation and valve surgery is a viable option for end-stage heart failure patients with valvular disease. This approach demonstrated good survival and VAD support rates in a recent study.

Background:

  • Mechanical circulatory support (MCS) use is expanding to sicker patients.
  • Management strategies for end-stage heart failure patients with valvular disease undergoing MCS are not well-defined.

Purpose of the Study:

  • To analyze the experience of combined ventricular assist device (VAD) implantation and valvular procedures in patients with valvular heart disease and end-stage heart failure.

Main Methods:

  • Retrospective review of 67 MCS patients from April 2007 to September 2011.
  • Identified 12 patients (17.9%) who underwent concomitant valvular procedures.
  • Procedures included aortic insufficiency repair, aortic valve replacement, mitral commissurotomy, and tricuspid valve repair.

Main Results:

  • All 12 patients were male, with a mean age of 50.3 years.
  • Survival rate was high at 91.6% (11/12) in early and late postoperative periods.
  • 83.3% (10/12) remained on VAD support, and one patient was bridged to transplantation.

Conclusions:

  • Combined valve surgery during VAD implantation is a reasonable strategy for select patients.
  • This approach shows promising outcomes in managing complex valvular heart disease in end-stage heart failure.
