Knee Joint

The knee joint is the most complicated joint in the body. It consists of three articulations– two tibiofemoral and one patellofemoral. As is characteristic of synovial joints, the knee joint has a thin articular capsule that partially surrounds this joint cavity. Summary

The number of total knee arthroplasties (TKA) doubled, but osteoarthritis severity and physical quality of life did not change. Mental quality of life scores improved, suggesting potential positive impacts on patient outcomes.

Background:

  • The incidence of primary total knee arthroplasty (TKA) in Denmark nearly doubled between 2004 and 2009.
  • This increase has led to speculation about whether indications for TKA have weakened.

Purpose of the Study:

  • To compare preoperative osteoarthritis severity and health-related quality of life (QoL) in patients undergoing primary TKA in 2004 versus 2009.
  • To investigate potential shifts in surgical indications for TKA.

Main Methods:

  • A comparative analysis of 154 TKAs in 2004 and 369 TKAs in 2009.
  • Random sampling of representative patient groups (n=44 in 2004, n=106 in 2009) for preoperative Short Form (SF)-36 questionnaire completion.
  • Assessment of Kellgren-Lawrence (K-L) osteoarthritis grades from preoperative radiographs.

Main Results:

  • No significant differences were observed in gender distribution or mean age at surgery between the two groups.
  • The proportion of patients with K-L grades 3-4 osteoarthritis was similar (52.4% in 2004 vs. 49.6% in 2009, p=0.57).
  • Preoperative physical component scores of the SF-36 showed no significant change (32.6 in 2004 vs. 33.7 in 2009, p=0.44), while mental component scores significantly increased in 2009 (43.0 in 2004 vs. 49.3 in 2009, p=0.003).

Conclusions:

  • Preoperative osteoarthritis severity and physical quality of life remained consistent between 2004 and 2009.
  • The findings suggest that the indications for TKA have not weakened based on these parameters.
  • An improved preoperative mental component score in 2009 may positively influence postoperative outcomes.
