Routine chest X-ray prior to thyroid surgery: is it always necessary?

Preoperative chest X-rays (CXRs) do not predict difficult intubation in thyroid surgery patients, even with identified tracheal deviation. Routine CXR for this purpose is not recommended.

- Patients with TD had larger thyroid glands and more tracheal compressive symptoms but did not experience significantly more difficult intubations or attempts.
- Incidental lung findings on CXR led to further investigation in 5% of patients, revealing additional pathology in only 1%.
Conclusions:
- There is no demonstrated correlation between preoperative chest X-ray findings of tracheal deviation and the difficulty of intubation in patients undergoing thyroid surgery.
- Routine preoperative chest X-ray for the sole purpose of identifying tracheal deviation in candidates for thyroid surgery is not clinically warranted.
