To report outcomes from a single surgeon using this technique.

Retrospective review of 92 eyelids in 66 patients with acquired blepharoptosis.
Patients underwent small-incision external levator advancement with uniform suture placement.
Data collected included margin-to-reflex distance (MRD) pre- and post-operatively.

Main Results:

Mean preoperative MRD was 0.70 mm, increasing to 2.95 mm postoperatively.
Eyelid symmetry ( < 1 mm MRD difference) was achieved in 74% of patients.
Bilateral ptosis repair showed higher symmetry rates (81%) compared to unilateral (70%).
Revision surgery was needed in 9 patients; only 2 minor complications occurred.

Conclusions:

The simplified suture placement method for small-incision external levator advancement is effective.
This technique provides a safe and efficient option for acquired ptosis correction.
The refined approach offers good functional and aesthetic outcomes.
