Main Results:

  • Mean preoperative MRD was 0.70 mm, increasing to 2.95 mm postoperatively.
  • Eyelid symmetry ( < 1 mm MRD difference) was achieved in 74% of patients.
  • Bilateral ptosis repair showed higher symmetry rates (81%) compared to unilateral (70%).
  • Revision surgery was needed in 9 patients; only 2 minor complications occurred.

Conclusions:

  • The simplified suture placement method for small-incision external levator advancement is effective.
  • This technique provides a safe and efficient option for acquired ptosis correction.
  • The refined approach offers good functional and aesthetic outcomes.
