Summary

The ProFile and Self-Adjusting File (SAF) system removed significantly more root filling residue than ProTaper retreatment files. While neither method achieved complete cleaning, the ProFile-SAF combination proved more effective for gutta-percha removal.

Background:

  • Rotary instruments are effective for root canal retreatment but often leave residual filling material.
  • Efficient removal of root filling residue is crucial for successful endodontic retreatment.

Purpose of the Study:

  • To evaluate the efficacy of ProTaper retreatment files followed by ProTaper F1/F2 instruments in removing gutta-percha.
  • To compare this efficacy with a ProFile instrument followed by the Self-Adjusting File (SAF).

Main Methods:

  • Twenty-eight mandibular molars with oval distal root canals were used.
  • Two groups were formed: ProTaper retreatment/ProTaper instruments and ProFile/#25 .06 followed by SAF.
  • High-resolution micro-computed tomography (micro-CT) was used to quantify residual root filling material after using chloroform.

Main Results:

  • The ProFile-SAF group left significantly less residue (median 0.41%) compared to the ProTaper group (median 5.39%).
  • 57% of teeth in the ProFile-SAF group met the 'effectively cleaned' threshold (<0.5% residue), versus 0% in the ProTaper group.
  • The ProFile-SAF procedure was also faster than the ProTaper protocol.

Conclusions:

  • Neither retreatment method completely eliminated all root filling residue.
  • The ProFile instrument combined with the SAF was more effective in reducing root filling residue than the ProTaper retreatment system.
