Background:

  • Thymic cysts are rare mediastinal masses, accounting for 5.6% of all mediastinal tumors and 35% of congenital cysts.
  • Eight cases treated between 1967 and 1989 highlight their incidental discovery via chest roentgenograms.

Observation:

  • Most patients were asymptomatic, with no associated autoimmune disorders like myasthenia gravis.
  • Imaging revealed characteristic findings: water density on CT, low T1, and high T2 signal intensity on MRI. Gallium scans were negative.

Findings:

  • All eight cases were surgically treated, diagnosed as benign thymic cysts with no signs of malignancy.
  • Post-operative follow-up showed no recurrence in any patient.

Implications:

  • Elevated CA19-9 and CEA levels in intracystic fluid, contrasting with normal serum levels, may aid preoperative diagnosis of thymic cysts.
  • Percutaneous aspiration can be a diagnostic and therapeutic option for poor-risk patients with non-malignant thymic cysts.
