Background:

  • Schnitzler syndrome is a rare autoinflammatory disorder characterized by fever, rash, and paraproteinemia.
  • Anakinra has shown efficacy but requires daily injections.
  • Canakinumab, an IL-1β inhibitor, offers potential for less frequent dosing.

Purpose of the Study:

  • To evaluate the efficacy and safety of canakinumab in a patient with Schnitzler syndrome.

Main Methods:

  • A single patient with Schnitzler syndrome received 150 mg of canakinumab subcutaneously every 8 weeks for 6 months.
  • Treatment was resumed upon symptom flare after initial discontinuation.

Main Results:

  • Canakinumab rapidly resolved fever, rash, and fatigue, and reduced C-reactive protein levels.
  • Symptoms recurred upon treatment interruption but were controlled upon resumption.
  • The treatment was well-tolerated with no adverse events.

Conclusions:

  • Canakinumab demonstrates potential as an effective and safe treatment for Schnitzler syndrome.
  • The extended half-life of canakinumab allows for convenient infrequent administration.
