Background:

Endoscopic thyroidectomy offers cosmetic advantages by avoiding neck scars.
A novel gasless endoscopic thyroidectomy technique using a single anterior chest wall incision was developed.

Purpose of the Study:

To evaluate the feasibility and outcomes of a novel gasless single-incision endoscopic thyroidectomy via an anterior chest wall approach.

Main Methods:

48 patients with benign thyroid nodules underwent the procedure between March 2009 and November 2010.
A 3-cm incision on the anterior chest wall was used, with dissection to the thyroid cartilage.
The Harmonic scalpel was used for resection, preserving key nerves and parathyroids.

Main Results:

The average operating time decreased significantly in the second half of the cases (112 minutes vs. 138 minutes).
Three cases required conversion to open surgery (2 for malignancy, 1 for bleeding).
Postoperative complications included transient hoarseness and hematoma; no wound infections occurred, and patients reported high satisfaction with cosmetic results.

Conclusions:

The anterior chest wall approach combined with the gasless technique makes single-incision endoscopic thyroidectomy feasible and practical.
This method provides excellent cosmetic results, enhancing patient satisfaction.
