  • To identify factors influencing the timely adoption of health interventions in resource-limited settings.

  • Screened World Health Organization (WHO) databases for regulatory approval and implementation start dates.
  • Employed descriptive analyses and statistical models (logistic regression, Cox proportional hazard) to assess milestone associations.
  • Categorized countries by World Bank income criteria and evaluated milestones: coordinating group, tailored intervention availability, financing commitment, and WHO recommendations (initial and comprehensive).

Main Results:

  • After 10 years, only 4% of low-income countries (LICs) implemented any of the studied vaccines; malaria interventions reached 37% of LICs.
  • Implementation rates varied, with some interventions mirroring the hepatitis B vaccine (HepB) and others showing faster or slower uptake.
  • International financing commitment and initial WHO recommendations were temporally linked to implementation; initial WHO recommendation was the sole significant factor across all analyses (RR=1.97, P<0.001).

Conclusions:

  • Milestones may not be achieved early enough in the R&D process to accelerate intervention implementation.
  • A framework is proposed to guide the development of future interventions, emphasizing critical milestone timing.
  • Optimizing milestone timing relative to R&D is crucial for accelerating the public health impact of new interventions.
