Background:

Observation:

  • A 61-year-old male presented with hypercalcemia due to an ectopic parathyroid adenoma.
  • Initial ultrasonography and scintigraphy suggested thyroid nodules, but failed to localize the mediastinal adenoma.
  • Post-thyroid surgery, persistent hypercalcemia indicated an undiagnosed ectopic adenoma.

Findings:

  • A CT scan revealed a 27 x 22 mm mass in the upper anterior mediastinum.
  • Histopathology confirmed the mass as a parathyroid adenoma.
  • Successful removal was achieved via a laterocervical approach.

Implications:

  • This case underscores the importance of considering ectopic parathyroid adenomas in cases of persistent hypercalcemia.
  • Advanced imaging like CT is crucial for localizing these challenging lesions.
  • Multidisciplinary diagnostic and surgical approaches are vital for successful management.
