Summary

Bilateral diaphragm paralysis can cause respiratory failure. Surgical diaphragm plication successfully weaned two patients from mechanical ventilation, with one showing further improvement after contralateral diaphragm plication.

  • Two cases of severe bilateral diaphragm paralysis requiring mechanical ventilation are presented.
  • These patients did not respond to intermittent non-invasive ventilation, necessitating alternative treatment strategies.

Findings:

  • Unilateral diaphragm plication successfully enabled mechanical ventilation weaning in both subjects.
  • One patient experienced additional improvement after contralateral diaphragm plication for the other paralyzed hemidiaphragm.
