Background:

Effective evaluation tools for physician consultations are limited.
Assessing telephone consultations presents unique challenges.
The 5Cs model (contact, communicate, core question, collaboration, closing the loop) was developed to address these gaps.

Purpose of the Study:

To establish validity evidence for the 5Cs model checklist.
To assess its utility in evaluating simulated emergent telephone consultations.
To provide a reliable tool for medical educators and supervisors.

Main Methods:

Checklist items were derived from a theoretical model and expert input.
Two simulated emergent cases (psychiatric and surgical) were developed.
Residents conducted telephone consultations, rated by faculty using the 5Cs checklist.

Main Results:

Content validity was established through expert consensus and literature review.
High interrater reliability (intraclass correlation = 0.94) was observed.
Strong internal structure validity (phi coefficients of 0.89 and 0.8) and correlations with global ratings (0.59-0.79) were found.

Conclusions:

The 5Cs model checklist is a valid and reliable tool for assessing telephone consultations.
Findings support its use in medical training and performance evaluation.
This checklist enhances the assessment of critical physician communication skills.
