Summary

Preoperative screening using the PFA-100 test in neurosurgery patients did not reduce bleeding complications. While desmopressin use increased, it did not significantly alter transfusion rates or bleeding events.

Area of Science:

  • Neurosurgery
  • Hematology
  • Medical Device Technology

Background:

  • Perioperative bleeding is a significant risk in neurosurgery.
  • Platelet function analyzers (PFA) are used to assess platelet function.
  • The PFA-100's utility in reducing bleeding complications in neurosurgical patients is not well-established.

Purpose of the Study:

  • To evaluate the effectiveness of the PFA-100 in preoperative screening for neurosurgical patients.
  • To determine if PFA-100 screening reduces perioperative bleeding complications.
  • To assess the impact of PFA-100 screening on desmopressin administration and transfusion rates.

Main Methods:

  • Prospective study comparing patients with and without PFA-100 screening.
  • Inclusion of neurosurgical patients with intracranial space-occupying lesions.

  • PFA-100 testing, desmopressin administration for abnormal results, and comparison of bleeding events and transfusions.
  • Main Results:

    • No significant difference in clinically relevant intracranial bleeding between PFA-100 screened and non-screened groups (2.2% vs 2.0%).
    • A significant increase in desmopressin administration in the PFA-100 group (14.0% vs 4.9%).
    • No significant difference in transfusion rates between the groups.

    Conclusions:

    • Preoperative PFA-100 screening in neurosurgical patients did not reduce perioperative bleeding complications.
    • PFA-100 screening led to increased desmopressin use without clinical benefit.
    • Routine PFA-100 testing may not be beneficial for preventing bleeding in this patient population.
