Background:

  • Intravascular lymphoma (IVL) is a rare hematologic malignancy characterized by the proliferation of lymphoid cells within blood vessels.
  • Clinical presentations of IVL are diverse and often nonspecific, making diagnosis challenging.

Observation:

  • A 59-year-old woman experienced syncope, followed by subacute cognitive decline culminating in a minimally conscious state.
  • Despite transfer to a tertiary care center, the patient unfortunately passed away shortly thereafter.

Findings:

  • Post-mortem brain autopsy confirmed the diagnosis of B-cell intravascular lymphoma.
  • The findings suggest that syncope may be an initial neurological symptom of central nervous system IVL.
