Background:

  • Small renal masses (SRMs) are increasingly detected, necessitating effective and kidney-sparing treatment options.

Purpose of the Study:

  • To evaluate the outcomes of a clampless laparoscopic partial nephrectomy (cl-LPN) technique.
  • To assess the impact of cl-LPN as an emerging treatment for small renal masses (SRMs).

Main Methods:

  • A prospectively maintained database of 117 patients undergoing LPN between January 2009 and December 2011 was reviewed.
  • Patients were divided into two groups: clampless LPN (cl-LPN) and conventional LPN with renal artery clamping.
  • Perioperative data, complications, and renal function (serum creatinine, eGFR) were compared between groups.

Main Results:

  • The clampless LPN group (41 patients) had significantly smaller tumors compared to the conventional LPN group (76 patients).
  • Warm ischemia time was 0 minutes for cl-LPN, versus a mean of 20.90 minutes for conventional LPN.
  • A significantly smaller postoperative decrease in estimated glomerular filtration rate (eGFR) was observed in the cl-LPN group (p=0.0445).

Conclusions:

  • Clampless LPN is a safe and effective technique for surgically treating SRMs.
  • This approach allows for the treatment of SRMs, including those in complex locations, without causing ischemic injury to the kidney.
  • Cl-LPN represents a valuable advancement in minimally invasive renal surgery.
