Background:

  • 18F-fluorothymidine (FLT) Positron Emission Tomography (PET) is utilized for assessing cell proliferation.
  • Distinguishing malignant from benign breast lesions is crucial for patient management.
  • Mediastinal masses require accurate diagnostic approaches.

Observation:

  • A 48-year-old woman with equivocal bilateral breast lesions underwent an 18F-FLT PET scan.
  • No FLT-avid breast lesions were detected.
  • An intensely FLT-avid lesion was incidentally found in the right anterior mediastinum.

Findings:

  • The mediastinal lesion showed only mild 18F-fluorodeoxyglucose (FDG) uptake on a subsequent PET scan.
  • Video-assisted thoracic surgery thymectomy was performed.
  • Histopathological analysis confirmed invasive thymoma.

Implications:

  • 18F-FLT PET imaging can incidentally detect thymoma, even when used for other indications.
  • The distinct uptake patterns of FLT and FDG may aid in diagnosing thymoma.
  • This case underscores the value of comprehensive PET imaging in unexpected findings.
