Background:

  • Alzheimer's disease (AD) typically spares primary sensory/motor cortex, lacking motor symptoms.
  • Presenile dementia cases often present with diverse clinical and pathological profiles.

Observation:

  • A case of presenile dementia with progressive hemiparesis is detailed.
  • Autopsy revealed significant somatosensory cortex pathology (plaques, tangles) in this patient.
  • Degeneration of the nucleus basalis and locus ceruleus was also observed.

Findings:

  • Moderate cortical cholinergic deficiency was detected.
  • Somatosensory cortex showed profuse Alz-50 antibody staining.
  • Somatostatin-like immunoreactivity remained normal, contrasting with typical AD findings.

Implications:

  • This case underscores the extreme variability of Alzheimer's disease presentation.
  • Clinical symptoms, pathological distribution, and neurochemical profiles in AD can differ significantly.
  • Further research is needed to understand the full spectrum of AD pathology and clinical manifestations.
