Summary

Traumatic hyphema patients, particularly black individuals, face a high risk of secondary hemorrhage. Hospitalization and aminocaproic acid treatment are recommended to reduce this risk and prevent ocular damage.

Background:

Purpose of the Study:

  • To investigate the incidence and risk factors for secondary hemorrhage in traumatic hyphema.
  • To evaluate the efficacy of aminocaproic acid in preventing secondary hemorrhage.

  • Retrospective review of 241 patients treated for traumatic hyphema between 1980 and 1989.
  • Analysis of patient demographics, initial hyphema severity, and treatment outcomes.

Main Results:

  • Secondary hemorrhage occurred in 19% of patients, with a significantly higher rate in black patients (P < .005).
  • Patients with initial hyphema filling <25% of the anterior chamber were more likely to develop secondary hemorrhage.
