Single-stage Dynamic Reanimation of the Smile in Irreversible Facial Paralysis by Free Functional Muscle Transfer

  • Reconstructions were generally satisfactory, with a low rate of complete extrusion (2.6%).
  • Common complications included wide scars (33.3%), hypertrophic scars (17.9%), and sagging (14.1%).
  • Other complications noted were partial necrosis, hematoma, lower lid ectropion, atrophy, sensory decrement, and infection.
  • Conclusions:

    • Tissue expansion allows for optimal aesthetic reconstruction by using similar adjacent skin, enabling interchangeable use of lower face or neck expanders.
    • This technique minimizes donor site morbidity, even when combined with free flaps, offering better color match for improved results.
