Development of the Oral Microbiota01:28

Development of the Oral Microbiota

The establishment of the oral microbiome begins before birth, challenging the long-held belief that the fetal oral cavity is sterile. The presence of oral microbes such as Streptococcus and Fusobacterium in amniotic fluid suggests that microbial exposure may occur in utero, potentially through translocation from the maternal oral or gastrointestinal tract. This early colonization primes the neonatal immune system and sets the stage for subsequent microbial succession. Maternal health,...

Healthcare Associated Infections II: Preventive Measures

Essential infection prevention measures are based on the knowledge of the infection chain, the modes of transmission in healthcare settings, and the use of the best practices in all healthcare settings. Compulsory public reporting of healthcare-associated infection rates is needed to allow individuals and the community to make informed choices regarding selecting a healthcare facility.
The best practices for preventing healthcare-associated infections include hand hygiene, patient risk...
Standard Precaution01:26

Standard Precaution

Standard precautions are the minimum infection control safeguards used while caring for all patients, irrespective of their disease condition. They help prevent the spread of common infectious microorganisms to healthcare workers, patients, and visitors in all healthcare settings.
Hand hygiene is the most crucial means to prevent the transmission of disease. Employers are legally required to provide their workers with personal protective equipment (PPE) to minimize exposure or contact with...
Teratogenicity01:07

Teratogenicity

The ability of a drug to produce structural deformations and functional abnormalities in the developing embryo or the fetus is called teratogenicity, and the drug producing this effect is known as a teratogen. Teratogenic effects include stillbirth, miscarriage, intrauterine growth restriction, and neurocognitive delay. A teratogen may affect the embryo at different stages of development, which is important in determining the type and extent of the damage. During blastocyst formation, the early...
Pathophysiology of Diabetes01:20

Pathophysiology of Diabetes

Diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia. The four categories of diabetes are type 1 diabetes, type 2 diabetes, other specific types of diabetes, and gestational diabetes.
Type 1 diabetes is characterized by autoimmune-mediated destruction of pancreatic β cells, with environmental factors potentially triggering this process in genetically susceptible individuals. Despite many not having a family history, certain genes increase susceptibility, suggesting a...
Guidelines for Writing Outcome01:11

Guidelines for Writing Outcome

When developing expected outcomes for a patient care plan, the nurse should adhere to the following recommendations:
Patient outcomes reflect the patient's response to the goal rather than what the nurse aims to achieve. Terminology should be observable and measurable to avoid the reader's interpretation. The desired outcome should be realistic and achievable in the designated care timeframe. Expected outcomes should align with adjunctive therapies. The outcome should enhance care evaluation by...

Summary

French perinatal guidelines had minimal impact on obstetric practices, with most changes predating guideline publication. Only active management of the third stage of labor showed significant practice improvement.

Area of Science:

  • Obstetrics and Gynecology
  • Public Health Policy
  • Clinical Practice Guidelines

Background:

  • French perinatal guidelines were issued without impact assessments.
  • This study evaluated the real-world effects of eight French perinatal guidelines on obstetric practices.

Purpose of the Study:

  • To assess the impact of French perinatal clinical practice guidelines on obstetric care.
  • To determine if guideline implementation led to changes in key perinatal indicators.

Main Methods:

  • A historical cohort study using the Audipog database (1994-2006) in France.
  • Analysis of 107,450 pregnancies, examining perinatal indicators before and after guideline publication.
  • Statistical comparison of standardized rates using Chi-squared tests.

Main Results:

  • Caesarean delivery rates steadily increased; exclusive breastfeeding and smoking cessation during pregnancy showed slow but consistent rises.
  • Active management of the third stage of labor significantly increased post-guideline.
  • Impacts on low-birth-weight infant delivery location, preterm labor management, and episiotomy rates were limited or fluctuated.

Conclusions:

  • Most clinical practice guidelines had a low impact on actual obstetric practices in France.
  • Observed practice changes often initiated before guideline release, suggesting other influencing factors.
  • Active management of the third stage of labor was an exception, demonstrating a positive impact.
