Background:

  • A 57-year-old patient with a history of tongue squamous cell carcinoma presented with persistent right ear pain.
  • Magnetic Resonance (MR) imaging of the neck revealed diffuse neoplastic infiltration of the right C2 root.

Findings:

  • The MR examination demonstrated perineural tumor spread.
  • Distal involvement of the great auricular nerve and vagus nerve was evident.
  • This case represents the first documented instance of cervical plexus involvement in head and neck cancer, detailing its interconnection with cranial nerves via the great auricular nerve.

Implications:

  • This finding expands the understanding of cancer spread pathways in head and neck malignancies.
  • Highlights the importance of considering perineural spread in patients with unexplained head and neck pain.
  • Suggests the need for detailed imaging protocols to evaluate cervical plexus involvement in relevant oncological cases.
