Background:

  • Diffuse large B-cell lymphoma (DLBCL) is an aggressive non-Hodgkin's lymphoma.
  • Skin involvement in DLBCL can manifest in various ways, sometimes preceding systemic diagnosis.
  • Early recognition of cutaneous manifestations is crucial for timely diagnosis and treatment.

Observation:

  • A 56-year-old woman presented with a two-month history of pruritic, erythematous plaques and nodules on her skin.
  • Clinical examination revealed hepatosplenomegaly, lymphadenopathy, and constitutional symptoms including night sweats and weight loss.
  • Laboratory findings included pancytopenia and elevated lactate dehydrogenase (LDH).

Findings:

  • Skin biopsy confirmed diffuse sheets of CD20 and CD79a positive neoplastic cells, indicative of DLBCL.
  • Computed tomography (CT) showed widespread lymphadenopathy.
  • Bone marrow biopsy confirmed bone marrow involvement by lymphoma.

Implications:

  • This case highlights the importance of considering DLBCL in patients with extensive cutaneous lesions and systemic symptoms.
  • Prompt diagnosis through skin biopsy and staging investigations is essential for effective management.
  • Chemotherapy demonstrated efficacy in achieving remission of both cutaneous and systemic disease.
