Background:

Purpose of the Study:

  • To determine the incidence and epidemiology of sequential hip fractures in the elderly.

Main Methods:

  • Data collected from 22 acute orthopaedic units in Scotland (1998-2005).
  • Analysis of data during acute hospital stay and at 120 days post-admission.
  • Time-period analysis included six-month intervals up to 8 years and twenty-day intervals for the first 2 years post-primary fracture.

  • The risk of sequential fracture was highest within the first year (3% of survivors), decreasing to 2% per year thereafter.
  • Survival to 12 months post-sequential fracture was 63%, compared to 68% for single fractures (p=0.03).

Conclusions:

  • Sequential hip fracture is an uncommon but serious injury in the elderly.
  • Patients experiencing sequential hip fractures demonstrate poorer survival rates and functional status.
  • Further research and targeted interventions are needed for this high-risk group.
