Levosimendan did not significantly impact platelet aggregation in rats during sepsis, despite in vitro findings suggesting otherwise. This study indicates levosimendan

Area of Science:

  • Pharmacology and Toxicology
  • Cardiovascular Research
  • Hematology

Background:

  • Levosimendan is used for heart failure during sepsis, but its effect on platelets remains controversial.
  • Sepsis can alter platelet function and number, impacting coagulation and patient outcomes.
  • Understanding levosimendan's interaction with platelets is crucial for optimizing sepsis treatment.

Purpose of the Study:

  • To investigate the short- and long-term effects of levosimendan on platelet aggregation in rats.
  • To determine if levosimendan influences platelet function in a sepsis model.
  • To compare in vivo and in vitro effects of levosimendan on platelet aggregation.

Main Methods:

  • Adult rats were divided into four groups: sham, levosimendan, lipopolysaccharide (LPS)-induced sepsis, and LPS + levosimendan.

  • Levosimendan was administered intravenously 24 hours after LPS injection.
  • Platelet aggregation was measured using the Dynabyte Multiplate system with adenosine diphosphate and collagen stimulation at 10 minutes and 5 days post-administration. Platelet counts were also determined.
  • Main Results:

    • LPS significantly reduced platelet aggregation one day post-injection, with a marked increase observed six days later, correlating with platelet counts.
    • In vivo administration of levosimendan (clinical dosage) showed no significant effect on rat platelet aggregation.
    • High-dose levosimendan demonstrated an effect on platelet function in vitro, contrasting with the in vivo findings.

    Conclusions:

    • The in vivo use of levosimendan does not significantly affect blood coagulation in this rat sepsis model.
    • Levosimendan's effects on platelets in vivo appear minimal, even with altered platelet counts during sepsis.
    • Clinical implications suggest levosimendan is safe regarding platelet function in sepsis patients under these conditions.
