Imaging Studies II: Ultrasonography

IntroductionUltrasonography, or renal ultrasound, is a noninvasive medical imaging technique that uses high-frequency sound waves to visualize the kidneys, ureters, bladder, and surrounding tissues.Indications for Urinary System UltrasonographyUrinary system ultrasonography is indicated in various clinical scenarios, such as:Kidney Stones (Urolithiasis): To detect and monitor the size and presence of kidney or urinary tract stones.Hydronephrosis: To assess the dilation of the renal pelvis and...

Imaging Studies V: Intravenous Urography and Retrograde Pyelography

IntroductionIntravenous Urography (IVU) and Retrograde Pyelography (RP) are important diagnostic imaging techniques used to evaluate the urinary system. These methods help identify structural abnormalities, obstructions, and functional issues in the kidneys, ureters, and bladder. Both procedures use iodine-based contrast media to enhance the visibility of urinary tract structures on X-ray images, though they differ in their methods and indications.1. Urologic Endoscopic Procedure: Cystoscopic Examination

Meaning of Cystoscopic Examination:Cystoscopy is an essential diagnostic tool in urology that is used to assess the structure and function of the genitourinary system. It provides a direct view of the urethra, bladder, and, in some cases, the ureteral openings. This procedure helps detect structural abnormalities, infections, cancers, and blockages in the urinary tract. There are two types of cystoscopy:Flexible cystoscopy is commonly performed in outpatient settings due to its less invasive...
Urodynamic Studies: Uroflowmetry01:19

Urodynamic Studies: Uroflowmetry

Uroflowmetry is a non-invasive urodynamic test designed to measure various aspects of urination, including volume, flow rate, and the time to void. This test is crucial for diagnosing and assessing conditions such as bladder outlet obstruction, bladder dysfunction, incomplete bladder emptying, incontinence, and urinary tract blockages caused by benign prostatic hyperplasia (BPH) and urethral strictures.Pre-Test Instructions:Before a uroflowmetry test, patients are typically advised to drink...
Measurement of Fluid Pressure01:16

Measurement of Fluid Pressure

Fluid pressure is commonly measured using devices called manometers, which rely on liquid columns to indicate pressure differences. The height of a liquid column in a manometer reflects the pressure exerted by the fluid, providing a simple yet effective means of measurement. Different types of manometers serve specific purposes based on their configurations and the type of fluids involved.
A basic form of manometer is the piezometer, a vertical tube open at the top and filled with the same...

Imaging Studies VI: Voiding Cystourethrography and Cystography

Voiding Cystourethrography (VCUG) and Cystography are specialized radiographic procedures used to examine the structure and function of the bladder and urethra.Voiding Cystourethrography (VCUG)A Voiding Cystourethrogram (VCUG) is a diagnostic imaging procedure that assesses the anatomy and function of the lower urinary tract. It focuses on the bladder, bladder neck, and urethra, helping detect abnormalities such as vesicoureteral reflux (VUR)—the backward or reverse flow of urine into the...

Summary

Measuring upper urinary tract pressure during ureteroscopy offers a practical, non-invasive method to assess obstruction severity. This technique aids in predicting kidney function prognosis following ureteral calculus treatment.

Area of Science:

  • Urology
  • Nephrology
  • Medical Devices

Background:

  • Ureteral calculi often cause upper urinary tract obstruction.
  • Accurate assessment of obstruction severity is crucial for predicting renal function.
  • Current methods may be invasive or lack precision.

Purpose of the Study:

  • To develop and validate a novel method for measuring intraluminal ureteral pressure during ureteroscopic procedures.
  • To assess the clinical significance and efficacy of this pressure measurement technique.

Main Methods:

  • A cohort of 44 patients with proximal ureteral calculi (acute and chronic obstruction) was studied.
  • Intraluminal ureteral pressure was measured using a non-invasive technique during ureteroscopy after calculus fragmentation.
  • Irrigant flow (0.9% sodium chloride) was used to transmit pressure readings.

Main Results:

  • Mean ureteral pressures were significantly higher in acute (74.5 mmHg) and chronic (32.5 mmHg) obstruction groups compared to controls (10.2 mmHg).
  • Ureteral pressure strongly correlated with obstruction duration, ureter diameter, and hydronephrosis depth.
  • Pressure showed a significant correlation with glomerular filtration rate (GFR) (r=0.975, P=0.001).

Conclusions:

  • The developed method for measuring intraluminal ureteral pressure is practical, reproducible, and non-invasive.
  • This pressure measurement serves as a valuable index for quantifying upper urinary tract obstruction severity.
  • The findings suggest this method can aid in predicting the prognosis of affected renal function.
