Area of Science:

  • Ophthalmology
  • Corneal Surgery
  • Endothelial Keratoplasty

Background:

  • Descemet stripping automated endothelial keratoplasty (DSAEK) is a common procedure for treating corneal endothelial dysfunction.
  • Standard DSAEK graft insertion often requires specialized instruments.
  • Exploring alternative, simpler insertion techniques is valuable for improving surgical efficiency.

Purpose of the Study:

  • To detail the manual no-fold push-in technique for DSAEK graft insertion.
  • To evaluate the safety and efficacy of this technique by analyzing postoperative outcomes.

Main Methods:

  • A series of twenty consecutive patients underwent DSAEK utilizing the manual no-fold push-in technique.
  • This method employs a 6-mm limbal wound, a Sheets glide, and a Sinskey hook, omitting the need for specialized inserters.

  • Standard surgical instrumentation was used throughout the procedure.
  • Main Results:

    • No primary graft failures were observed in the study cohort.
    • Three patients (15%) experienced graft dislocation requiring re-bubbling.
    • Average follow-up was 13.22 months, with an average endothelial cell loss of 23.31%.

    Conclusions:

    • The manual no-fold push-in technique is a straightforward and effective method for DSAEK graft insertion.
    • This technique utilizes readily available surgical tools, simplifying the procedure.
    • The study demonstrates that manual graft insertion can yield excellent postoperative results in DSAEK surgery.
