Background:

  • Crohn's disease is a chronic inflammatory condition that can affect various organs.
  • Dacryoadenitis, inflammation of the lacrimal gland, can present acutely and may be associated with systemic inflammatory diseases.
  • Patients with inflammatory bowel disease are at increased risk for certain autoimmune and inflammatory complications.

Observation:

  • A 41-year-old woman with a history of Crohn's disease developed acute dacryoadenitis with associated orbital inflammation.
  • Initial presentation involved painful swelling, conjunctival injection, and limited eye movement, with CT revealing lacrimal gland abscess.
  • A subsequent episode of dacryoadenitis responded poorly to antibiotics but resolved rapidly with oral corticosteroids.

Findings:

  • The patient's initial dacryoadenitis, complicated by abscess formation, required a prolonged course of intravenous and oral antibiotics for resolution.
  • Recurrent dacryoadenitis in the same patient demonstrated a significant lack of response to oral antibiotics.
  • Prompt administration of oral steroids led to rapid symptom resolution within 48 hours, with no recurrence after tapering.

Implications:

  • This case suggests that dacryoadenitis in Crohn's disease patients may have an inflammatory rather than purely infectious etiology, particularly in recurrent cases.
  • Corticosteroids may be a crucial therapeutic option for managing dacryoadenitis in immunocompromised patients with inflammatory bowel disease.
