  • Gallstone ileus is a rare but serious complication, occurring in 0.3-0.5% of cases.
  • High morbidity and mortality are associated with gallstone ileus due to comorbidities, advanced patient age, and delayed diagnosis.

Observation:

  • A case of partial colon obstruction caused by a large gallstone.
  • The gallstone was lost during an emergency laparoscopic cholecystectomy.
  • The calculus eroded the intestinal wall, leading to partial lumen occlusion and recurrent subocclusive episodes.

Findings:

  • The eroded gallstone caused partial colon obstruction and recurrent symptoms.
  • An intraperitoneal abscess formed and spontaneously drained through a subhepatic drain.
  • A persistent intermittent fistula became apparent after drain removal.

Implications:

  • This case highlights a rare surgical complication of gallstone disease.
  • Delayed diagnosis and management contribute to poor outcomes in gallstone ileus.
  • The formation of a fistula presents unique challenges in patient management.
