Summary

This study on acute myelopathy found that nearly half of patients had unilateral spinal cord disease. Spinal shock at presentation was linked to poor outcomes in these idiopathic acute myelopathy cases.

Background:

  • The clinical spectrum of acute intrinsic spinal cord disease is not well-defined due to inconsistent diagnostic criteria across studies.

Purpose of the Study:

  • To describe the clinical characteristics of patients with idiopathic acute myelopathy.
  • To identify factors associated with prognosis in acute myelopathy.

  • Retrospective analysis of 17 patients with idiopathic acute myelopathy, selected from a larger cohort of 34 patients with acute intrinsic myelopathies.
  • Review of clinical data, neuroimaging, cerebrospinal fluid analysis, and patient outcomes.

  • A significant proportion (47%) of patients exhibited predominantly unilateral spinal cord involvement.
  • A higher incidence (24%) of subsequent multiple sclerosis development was observed compared to previous reports, potentially due to broader inclusion criteria.
  • Elevated cerebrospinal fluid protein levels on repeat lumbar puncture were noted in patients presenting with spinal shock.
  • Spinal shock at presentation was strongly correlated with a poor patient outcome.

  • Broader diagnostic criteria may influence the observed rate of secondary multiple sclerosis.
