Aging and its effect on bone remodeling is the most common cause of bone disorders. In young and healthy people, bone deposition and resorption happen at an equal rate to maintain optimal bone health.
Bone deposition is also affected by the levels of sex hormones like estrogen and testosterone that promote osteoblast activity and bone matrix synthesis.   • To assess the relationship between implant overload and bone/implant loss.

  • Comprehensive database searches (PubMed, OVID, EMBASE, LILACS) up to December 2011.
  • Inclusion of studies on clinical, radiographic, or histological outcomes of overloaded implants.
  • PICO question: effect of overload vs. no overload on bone/implant loss in stable implants.

Main Results:

  • Most clinical studies (16) and some animal studies (25) were excluded due to high risk of bias.
  • Animal studies showed overload did not cause bone loss without inflammation; it was even anabolic.
  • Overload aggravated peri-implant tissue breakdown in the presence of inflammation.

Conclusions:

  • Evidence linking implant overload to bone/implant loss in well-integrated implants is limited and biased.
  • Overload alone did not negatively impact osseointegration in uninflamed conditions.
  • Inflammation significantly increased overload-induced bone resorption in animal models.
