Weaknesses in guideline development processes hinder health service delivery in Uganda. Improving guideline creation and dissemination is crucial for effective healthcare implementation.
Area of Science:
- Health Services Research
- Health Policy and Management
- Implementation Science
Background:
- Guidelines are essential for directing health service delivery but their implementation is often hindered by development process weaknesses.
- This study examines the development processes of health planning, services management, and clinical guidelines in Uganda's health sector.
- The research aims to understand how these processes impact guideline utility.
Purpose of the Study:
- To describe the processes of developing health planning, services management, and clinical guidelines in Uganda.
- To identify factors within guideline development that facilitate or impede their utility in health service delivery.
Main Methods:
- A mixed-methods approach combining qualitative and quantitative research was employed.
- Data were collected from the central Ministry of Health, district level, and service delivery points.
- Methods included document review, observations, key informant interviews, and guideline counts, with analysis using thematic content analysis and Microsoft Excel.
Main Results:
- 137 health sector guidelines were identified, with many related to Millennium Development Goals programs.
- Significant issues included content duplication, conflicting guidelines, inadequate consultation, and formats not suited for service delivery.
- Dissemination was not systematic, with over 60% of central guidelines not reaching service delivery points, and a lack of monitoring and evaluation frameworks.
Conclusions:
- The guideline development process significantly compromises guideline effectiveness in Uganda.
- Enhancing guideline quality requires concerted efforts at both country and regional levels, including resource pooling and capacity building.
- Establishing and enforcing robust systems and guidance for guideline development is essential for countries.
