Background:

  • Predicting long-term disability in multiple sclerosis (MS) remains a challenge in clinical practice.
  • Existing data predominantly comes from Northern Hemisphere cohorts, with limited information from South America.
  • Genetic, environmental, and racial factors may contribute to MS outcome heterogeneity.

Purpose of the Study:

  • To investigate factors influencing the long-term disability progression in a South American multiple sclerosis (MS) cohort.
  • To identify specific clinical predictors of disability milestones, such as reaching an Expanded Disability Status Scale (EDSS) of 6, 7, or 8.

Main Methods:

  • A cohort of 197 patients attending an MS Center was evaluated.
  • Clinical and demographic data were collected.
  • Survival analysis using Cox regression and Kaplan-Meier methods analyzed time to EDSS 6, 7, and 8.

Main Results:

  • Median time to EDSS 6 was 25.83 years; median time to EDSS 7 was 36.25 years.
  • Male sex, motor symptoms at onset, and brainstem symptoms at onset significantly increased the risk of reaching EDSS 6 and 7.
  • Frequent relapses within the first 5-10 years of disease onset were associated with an increased risk of reaching EDSS 8.

Conclusions:

  • Male patients with MS, particularly those with early motor/brainstem symptoms and frequent relapses, require vigilant monitoring.
  • These high-risk patients should be closely observed for early signs of treatment failure to optimize management strategies.
