Summary

Laparoscopic right anterior sectionectomy (Lap-RAS) is a feasible surgical option for select liver diseases. While technically demanding, this minimally invasive approach offers potential benefits for patients with benign and malignant conditions.

Area of Science:

  • Hepatobiliary Surgery
  • Minimally Invasive Surgery
  • Surgical Oncology

Background:

  • Laparoscopic major hepatectomy is established, but laparoscopic right anterior sectionectomy (Lap-RAS) remains infrequently reported.
  • This study details three cases of totally Lap-RAS, highlighting essential technical maneuvers.

Observation:

  • Case 1: 40-year-old male with intrahepatic bile duct stones and dilatation (suspected Caroli's disease).
  • Case 2: 63-year-old female with hepatocellular carcinoma in the liver.
  • Case 3: 54-year-old female with a hepatic mass and positive hepatitis B viral markers.

Findings:

  • Operative times ranged from 540 to 680 minutes.
  • Estimated blood loss varied from 120 to 500 mL, with no intraoperative transfusions required.
  • Patients were discharged between postoperative Days 7 and 30.

Implications:

  • Lap-RAS is a viable technique for carefully selected patients with liver conditions.
  • The procedure demands advanced surgical skills and meticulous technique.
  • Further research may expand the application of Lap-RAS in liver surgery.
