Background:

  • Deep-seated warts, particularly myrmecia on the sole of the foot, can present with significant pain and functional impairment.
  • Bone erosion is an uncommon but recognized complication of chronic or deep-seated lesions.

Observation:

  • A case of a deep palmo-plantar wart (myrmecia) was investigated.
  • The wart exhibited unusual depth and adherence to the underlying structures.
  • Radiographic or clinical examination revealed erosion of the phalanx beneath the wart.

Findings:

  • The myrmecial wart directly caused osteolytic changes and erosion of the underlying phalangeal bone.
  • Histopathological examination confirmed the presence of HPV within the wart tissue.
  • The extent of bone erosion correlated with the size and depth of the wart.

Implications:

  • This case underscores the potential for common cutaneous warts to cause significant underlying bone pathology.
  • Early recognition and appropriate management of deep palmo-plantar warts are crucial to prevent complications like bone erosion.
  • Further research into the mechanisms of wart-induced osteolysis may inform novel therapeutic strategies.
