Peripheral Artery Disease V: Postoperative Nursing Management

During the postoperative period, it is crucial to focus on maintaining circulation, identifying and managing potential complications, and planning for discharge.Nursing AssessmentVital signs monitoring: Regularly monitor vital signs, including blood pressure, heart rate, respiratory rate, and temperature, to detect early signs of complications such as bleeding and infection.Circulation assessment: Monitor pulses, perform Doppler assessments, and check capillary refill, color, temperature, and...

Total knee arthroplasty (TKA) improved pain and function in knees with prior patellectomy. However, functional outcomes were lower compared to knees with intact patellae, despite comparable WOMAC scores.

Area of Science:

  • Orthopedic surgery
  • Biomechanical engineering

Background:

  • The patella is crucial for knee extensor mechanism leverage.
  • Patellectomy does not prevent tibiofemoral arthrosis development.

Purpose of the Study:

  • To assess clinical outcome improvements after total knee arthroplasty (TKA) in patellectomized knees.
  • To compare TKA outcomes in patellectomized knees versus those with intact patellae.

Main Methods:

  • Retrospective analysis of 52 TKAs in patellectomized knees and 52 matched controls with intact patellae.
  • Comparison of preoperative and postoperative SF-12, WOMAC, and Knee Society scores (KSS).
  • Minimum follow-up of 24 months (mean 69 months).

Main Results:

  • Both groups showed significant improvements in WOMAC and KSS scores post-TKA.

Related Experiment Videos

  • Mean postoperative WOMAC scores were comparable between groups.
  • Mean KSS was lower in the patellectomized group compared to the control group.
  • SF-12 scores showed no significant difference between groups post-TKA.
  • Conclusions:

    • TKA effectively relieves pain and restores function in patellectomized knees with tibiofemoral arthrosis.
    • Functional outcomes (KSS) after TKA are generally lower in patients with a history of patellectomy compared to those with intact patellae.
    • Despite mechanical disadvantage, TKA is a viable option for managing tibiofemoral arthrosis in patellectomized knees.
