Area of Science:

  • Craniofacial surgery
  • Orthognathic surgery
  • Maxillofacial reconstruction

Background:

  • Midface hypoplasia requires complex 3D repositioning.
  • Craniofacial dysostosis presents significant skeletal challenges.
  • Le Fort osteotomies are key procedures for midface correction.

Observation:

  • Two cases of craniofacial dysostosis were treated.
  • Case 1: Simultaneous Le Fort I and Le Fort III osteotomies without pterygomaxillary disjunction.
  • Case 2: Modified Le Fort III osteotomy with median disjunction and reduction of facial dimensions.

Findings:

  • Combined Le Fort III and Le Fort I osteotomy (Obwegeser technique) allows simultaneous midface and dental repositioning.
  • Le Fort I without pterygomaxillary disjunction achieved midface mobilization preserving occlusal ratios.

  • Modified Le Fort III with median disjunction addressed transverse and pyriform dimensions.
  • Implications:

    • This combined technique offers a viable option for midface advancement in adults with normal occlusion.
    • Preserves dento-skeletal relations for improved functional and aesthetic outcomes.
    • Highlights the versatility of Le Fort osteotomies in treating complex craniofacial deformities.
