Infiltrating spinal angiolipomas represent a particularly rare subtype, posing diagnostic and surgical challenges.

A 58-year-old male presented with a 7-month history of progressive lower extremity weakness and sensory disturbances.
Magnetic resonance imaging revealed a well-enhanced mass infiltrating the T4-5 vertebral foramen.

Findings:

Surgical resection of the T4-5 mass was performed.
Histopathological examination confirmed the diagnosis of spinal angiolipoma.

Implications:

This case highlights the importance of considering rare spinal tumors in the differential diagnosis of progressive neurological deficits.
