Background:

  • Conventional tissue array techniques have limitations in representing tissue heterogeneity and preserving tissue integrity.
  • These limitations hinder comprehensive analysis of pathobiologic events and genomic aberrations.
  • Thin specimens and needle biopsies pose particular challenges for traditional methods.

Purpose of the Study:

  • To demonstrate the noninferiority and superiority of the novel Spiral Array technique compared to conventional tissue array.
  • To evaluate the Spiral Array's ability to improve donor tissue representation while maintaining architectural details.
  • To compare the morphologic features and diagnostic accuracy of both techniques.

Main Methods:

  • Spiral Array and conventional tissue array were created for 25 lung adenocarcinomas and 50 multiple tumors.
  • Tissue heterogeneity was assessed using cytokeratin 7 and epidermal growth factor receptor (EGFR) immunohistochemistry.
  • Morphologic preservation was evaluated by pathologists' accuracy in predicting histopathologic diagnosis and organ type.

Main Results:

  • Spiral Array demonstrated superior representation of EGFR staining intensity variations (P = .01).
  • Organ type prediction accuracy was significantly higher with Spiral Array (P = .047).
  • Histologic diagnosis prediction accuracy did not significantly differ between the two methods.

Conclusions:

  • Spiral Array offers significant advantages in capturing tissue heterogeneity.
  • The Spiral Array technique provides superior morphologic preservation compared to conventional methods.
  • These benefits enhance the utility of tissue arrays in research and diagnostics.
