Augmented reality (AR) guidance significantly improves mitral valve repair by enhancing transesophageal echocardiography (TEE). This novel approach offers safer, more accurate, and faster tool navigation for minimally invasive cardiac surgery.
Area of Science:
- Cardiovascular Surgery
- Medical Imaging
- Surgical Navigation Systems
Background:
- Minimally invasive mitral valve (MV) repair techniques are advancing.
- Current methods often rely on transesophageal echocardiography (TEE) alone, posing challenges in tool visualization and guidance due to limited resolution.
Purpose of the Study:
- To evaluate an augmented reality (AR) guidance system integrated with TEE for off-pump beating heart MV repair.
- To compare the safety and efficacy of AR-guided navigation versus TEE alone using the NeoChord DS1000 system.
Main Methods:
- An AR environment was created using a magnetic tracking system and sensors, overlaying virtual intracardiac landmarks onto biplane TEE images.
- The system was tested in a porcine model for MV repair with chordal reconstruction using the transapically delivered NeoChord DS1000.
Main Results:
- AR guidance resulted in successful tool navigation in all 12 attempts, compared to 9 of 12 with TEE alone.
- AR-guided navigation showed significantly reduced distance errors (5.2 vs 16.8 mm), navigation times (16.7 vs 92.0 seconds), and path lengths (225.2 vs 1128.9 mm).
- The risk of injury to other intracardiac structures was nearly 40-fold lower with AR, and it appeared to shorten the learning curve for novice surgeons.
Conclusions:
- Augmented reality-enhanced TEE provides more direct and safer intracardiac navigation for MV repair tools.
- AR imaging guidance demonstrated a fourfold improvement in accuracy, fivefold reduction in navigation time, and enhanced overall safety compared to TEE alone.
