Background:

  • Plastic bronchitis is a rare condition characterized by the formation and expectoration of bronchial casts.
  • While typically seen in pediatric patients, it can occur at any age.

Observation:

  • An 81-year-old male patient in intensive care presented with symptoms of plastic bronchitis type I.
  • The patient experienced profuse expectoration of wormlike, reddish-brownish material resembling bronchial casts.

Findings:

  • Histological examination revealed the expectorated material to be a slimy purulent secretion.
  • Abundant fibrin and blood were present, along with evidence of cytopathic effect from herpes virus.

Implications:

  • This case expands the understanding of plastic bronchitis, particularly in the elderly population.
  • The identification of herpes virus cytopathic effect suggests a potential role for viral infections in the pathogenesis of plastic bronchitis.
  • Further research is needed to elucidate the unclear pathogenesis of plastic bronchitis.
