Summary

Magnetic resonance (MR) imaging has low sensitivity for detecting cancer spread to the cerebrospinal fluid (CSF), even with gadolinium contrast. Gadolinium enhancement improves detection, especially in the spine, but overall accuracy remains limited for leptomeningeal metastases.
