  • No significant difference in ICU or hospital stay between groups.
  • Higher rates of hemoperitoneum reoperation and perihepatic hematomas in the drained group.
  • Increased need for postoperative paracentesis in the undrained group, particularly with preoperative ascites > 1000 mL.
  • Drained patients showed higher incidence of perihepatic hematomas and greater blood product consumption.

Conclusions:

  • OLT without intra-abdominal drainage is safe and does not elevate morbidity.
  • Intra-abdominal drains may contribute to hematoma formation and increased blood product usage.
