Summary

This study found no link between corticosteroid use and open-angle glaucoma (OAG) in the elderly. Different steroid types, including ophthalmic, inhaled, nasal, oral, and ointments, did not increase OAG incidence in this population.

Background:

  • The role of corticosteroid-induced open-angle glaucoma (OAG) in the general population remains unclear.
  • It is unknown if OAG is limited to susceptible individuals or a significant public health concern.

Purpose of the Study:

  • To investigate the association between corticosteroid use and the incidence of OAG in an elderly population.
  • To determine if various forms of steroid administration contribute to OAG development.

Main Methods:

  • Prospective population-based cohort study of 3,939 participants aged 55+ from the Rotterdam Study.
  • Ophthalmic examinations and continuous monitoring of corticosteroid use (ophthalmic, inhaled, nasal, oral, ointments) over approximately 10 years.
  • Logistic regression models were used to assess the association between steroid use and incident OAG, adjusting for relevant covariates.

Main Results:

  • During a mean follow-up of 9.8 years, 2.8% of participants developed OAG.
  • No statistically significant association was found between the use of ophthalmic, inhaled, nasal, oral steroids, or steroid ointments and the incidence of OAG.
  • Odds ratios for OAG ranged from 0.70 to 1.26 across different steroid classes, with p-values generally above 0.05.

Conclusions:

  • This study found no evidence that corticosteroid use, across various administration routes, is associated with an increased incidence of open-angle glaucoma in the elderly population.
  • The findings suggest that corticosteroid-induced OAG may not be a significant contributor to the overall burden of OAG in the general elderly population.
