Cimetidine does not significantly affect digoxin pharmacokinetics in patients with heart failure. This study found no significant differences in digoxin levels or elimination when combined with cimetidine.
Area of Science:
- Pharmacology
- Clinical Pharmacy
- Cardiology
Background:
- Cimetidine, a histamine H2-receptor antagonist, is widely used for acid-related disorders.
- Cimetidine is known to interact with various medications, potentially affecting drugs with a narrow therapeutic index.
- Previous reports on cimetidine-digoxin interaction are conflicting.
Purpose of the Study:
- To investigate the pharmacokinetic interaction between cimetidine and digoxin in patients with congestive heart failure.
- To determine if cimetidine alters the disposition of chronically administered digoxin.
Main Methods:
- Six hospitalized patients with stable congestive heart failure were enrolled.
- Patients received their standard digoxin maintenance dose.
- Cimetidine was administered after steady-state digoxin concentrations were confirmed.
Main Results:
- Analysis of digoxin serum concentrations showed no significant changes.
- Area under the curve (AUC) for digoxin remained comparable.
- The rate of digoxin elimination was not significantly different when cimetidine was co-administered.
Conclusions:
- Cimetidine does not appear to significantly alter the pharmacokinetic disposition of chronic digoxin therapy in patients with congestive heart failure.
- These findings suggest a lack of clinically significant interaction between cimetidine and digoxin in this patient population.
