Summary

Stress-induced severe atrio-ventricular block (A-V B) was identified in patients with stress symptoms. Electrophysiological study located the block, suggesting dual-chamber pacing as the optimal treatment.

  • Sixteen patients, many with stress symptoms, developed severe A-V block during stress testing, despite normal resting ECGs.
  • Electrophysiological studies localized the A-V block to either intra-hissian or infra-hissian regions.
  • Intra-hissian block occurred in 8 patients (7 with narrow QRS, 1 with wide QRS), while infra-hissian block occurred in 8 patients (all with wide QRS).
