Summary

Opportunities for residents to gain clinical experience in intensive care units (ICUs) significantly decreased over a nine-year period. This decline in patient admissions and procedures may impact future physician competency.

Background:

Clinical learning is vital for residency training.
The impact of work hour limitations on experiential learning is unclear.
Quantifying changes in clinical experience opportunities is necessary.

Purpose of the Study:

To quantify changes in clinical experience opportunities for residents in medical-surgical intensive care units (ICUs) over time.
To assess the impact of postgraduate medical education changes on resident learning.

Main Methods:

Retrospective analysis of electronic medical records from three academic medical-surgical ICUs (2001-2010).
Data included daily patient admissions and invasive procedures.
Resident opportunities for patient admission and procedures were calculated and compared over time.

Main Results:

A 32% decrease in opportunities to admit patients.
A 34% decrease in opportunities to perform procedures.
Both decreases were statistically significant (p < 0.001).

Conclusions:

Significant decline in resident clinical experience in ICUs over nine years.
Further research is needed to understand the impact on future physician performance.
Investigate how these changes affect residents transitioning to independent practice.
