Summary

Prospective memory tests are more sensitive for detecting cognitive changes in amyotrophic lateral sclerosis (ALS) patients than standard cognitive assessments. These tests can identify subtle deficits indicative of frontotemporal dysfunction in early-stage ALS.

Background:

  • Cognitive impairment is common in amyotrophic lateral sclerosis (ALS).
  • A sensitive screening method for ALS-related cognitive deficits is needed.
  • Frontotemporal dysfunction is increasingly recognized in ALS.

Purpose of the Study:

  • To identify a sensitive screening battery for subtle cognitive deficits in ALS patients.
  • To evaluate the utility of prospective memory tests in ALS cognitive assessment.

Main Methods:

  • Eighty ALS patients and 57 controls completed cognitive tests.
  • Tests included Mini-Mental Status Examination (MMSE), verbal fluency (VFT), Stroop Color Word Interference Test (CWT).
  • Prospective memory (PM) tests, event-based (EBPM) and time-based (TBPM), were administered.

Main Results:

  • No significant differences were found in MMSE, VFT, or CWT between ALS patients and controls.
  • Significant differences were observed in PM tests (EBPM: P=0.043; TBPM: P<0.001).
  • Time-based PM (TBPM) demonstrated higher sensitivity than event-based PM (EBPM), especially in early-phase patients.

Conclusions:

  • Prefrontal lobar dysfunction is present in ALS patients.
  • Cognitive deficits in ALS may originate in medial and spread to lateral prefrontal regions.
  • Prospective memory tests are more sensitive than traditional measures for detecting frontotemporal dysfunction in ALS.
