Summary

Single-incision laparoscopic surgery (SILS) for gallbladder removal is safe and feasible. With experience, operative times for SILS cholecystectomy become comparable to conventional laparoscopy (LC).

Background:

  • Single-incision laparoscopic surgery (SILS) offers a minimally invasive approach for gallbladder disease.
  • Initial experience with SILS cholecystectomy was compared to conventional four-trocar laparoscopic cholecystectomy (LC).

Purpose of the Study:

  • To evaluate the initial clinical experience and outcomes of SILS cholecystectomy.
  • To compare SILS cholecystectomy with conventional LC in terms of operative time, complications, and feasibility.

Main Methods:

  • Sixty SILS cholecystectomies were performed between May 2009 and April 2010.
  • Patients undergoing SILS were matched by Body Mass Index (BMI) with 60 patients who underwent conventional LC.

Main Results:

  • SILS cholecystectomy initially had longer operative times (55 min) compared to LC (49 min).
  • After the first 15 cases, SILS operative times decreased to 51 min, comparable to LC.
  • Wound infection rates were similar between SILS (3 patients) and LC (2 patients). No conversions to open surgery were required.

Conclusions:

  • SILS cholecystectomy is a safe and feasible option when performed by experienced surgeons.
  • Complication rates for SILS are comparable to conventional LC.
  • A learning curve of 10-15 procedures allows SILS operative times to approach those of conventional LC.
