Bone Remodeling

Bone remodeling is a continuous and balanced process of bone resorption by osteoclasts and bone formation by osteoblasts. In adults, it helps maintain bone mass and calcium homeostasis. While mechanical stress can stimulate turnover as part of the normal maintenance and reparative process, several hormones also regulate bone remodeling.

Purpose of the Study:

  • To evaluate the efficacy of the block tenting technique for vertical and horizontal alveolar ridge defect reconstruction.
  • To assess bone augmentation achieved with different intraoral bone block sources and bone substitutes.
  • To identify complications and contributing factors associated with the block tenting procedure.

Main Methods:

  • Retrospective analysis of 102 patients undergoing block tenting between 2005 and 2010.
  • Utilized intraoral bone blocks (ramus, chin, tuberosity) or allogeneic blocks, augmented with bone substitutes +/- PRGF.
  • Simultaneous or delayed implant placement, with data collected on augmentation, complications, and outcomes.

Main Results:

  • Significant horizontal augmentation in the anterior maxilla (4.31±0.93 mm) and vertical augmentation in the posterior maxilla (5.75±2.22 mm).
  • Ramus and allogeneic blocks yielded greatest horizontal gain; tuberosity blocks and combined materials provided greatest vertical gain.
  • Plasma rich in growth factors (PRGF) showed no significant effect; common complications included hematoma and inflammation. Graft failure occurred in 13 patients, primarily with allogeneic blocks.

Conclusions:

  • The block tenting technique demonstrates effectiveness for localized alveolar ridge augmentation.
  • This method can potentially decrease the reliance on autogenous bone grafts from donor sites.
  • Further research may optimize material selection and technique to minimize complications.
