Background:

Paragangliomas are rare neuroendocrine tumors typically arising from the adrenal medulla or sympathetic ganglia.
Bladder paragangliomas are exceptionally rare, often presenting with nonspecific symptoms, leading to diagnostic challenges.

Observation:

A 32-year-old male presented with dyspnea on micturition and hematuria.
Imaging revealed a left bladder wall polyp, confirmed as a submucosal pulsating mass.
Elevated plasma free metanephrines suggested a paraganglioma, despite normal urinary catecholamines and MIBG scan.

Findings:

The bladder mass was diagnosed as a paraganglioma.
The patient underwent partial cystectomy for complete tumor excision.
Post-operative pathology confirmed a completely excised paraganglioma.
